INSTRUCTIONS
- This is not a test, so there are no right or wrong answers to this survey.
- All of the questions should be answered by clicking one of the answer spaces. If you do not find an answer that fits exactly, use the one that comes closest. If any question does not apply to you, or you are not sure of what it means, just leave it blank.
- In the event you experience network congestion, your responses may take up to 90 seconds to register and display your next page of questions.
- In the event your browser stops registering (or takes longer than 3 minutes to respond after submitting your survey page or crashes), simply restart the survey and page up to the location that your browser stop responding.
- In the event you run out of classroom time, simply page all the way to the end of the survey.
